Once you purchase this evaluation, a log-in e-mail will be sent to you allowing you to log-in and fill out all the evaluation forms.  Once completed, these forms will be forwarded to our Naturopathic Physician and her support staff for review.  You will then receive by e-mail all her suggestions.
